Please DO NOT fill out this registration form until you have all of the STUDENT names that you will be submitting to audition. It  is not necessary for Teachers to pre-register. But, you must be a member of NMACDA. Please submit this form by September 27th, 2010. Audition recordings should be postmarked no later than October 4th, 2010. BE SURE to scroll down to the STUDENT NAME section and enter information for each student that will be auditioning. Click ADD ITEM for each student, BE SURE TO TYPE IN THE IMAGE VERIFICATION AND CLICK SUBMIT AFTER YOU HAVE FINISHED ENTERING ALL OF YOUR STUDENTS.

Include pertinent information such as name of school, department, classroom number, etc.

Include extra information such as apartment number.

Children's Choir: Use Soprano or Alto.
Youth Choir: Use Soprano, Alto, Tenor or Bass.

Type the student name as it would appear on your audition cd's and in a program.
