Teachers will need to register using this form for all Youth/Children All-State Choir Participants that were selected for the 2012 NMACDA choirs. 

Step 1: Re-enter all teacher and school information

Step 2: Enter all student information including all address, phone..etc. for the students who will be participating in the 2012 NMACDA All-State Choirs.

Reminder: Check names for spelling and voice part accuracy on your registration form and on the audition result lists. We will not make any changes to spelling after November 15, 2011.

Step 3: Send check or P.O. to our NMACDA treasurer $35.00 per student. (No refunds).

Payment needs to be received by December 1, 2011 in order to participate in the All-State Choirs.

Children's Choir: Use Soprano or Alto.
Youth Choir: Use Soprano, Alto, Tenor or Bass.

Type the student name as it would appear on your audition cd's and in a program.
