Student Registration Form

This information will be used for the program and for emergency purposes. All areas must be filled in before submitting.

Follow These Steps:

1. Fill out the TEACHER information section. E-mail address is required!

2. Fill out the STUDENT information section for you first student. PO Boxes should be entered in the Home Address Details box.

3. Click appropriate CHOIR and VOICE PART ASSIGNED.

4. Click ADD ITEM.

5. Repeat the process for each student.

6. When you are done entering students, enter the IMAGE VERIFICATION CODE and click SUBMIT APPLICATION.

7. You will receive an e-mail verification of you registration.

8. Remit your Registration Fees to the Treasurer, Brian Uerling. Registration is not complete until the fee is received by the Treasurer.

You must send a $35.00 participation fee for each student registered. Please make only one check or purchase order for all selected students from your group. Make checks or purchase orders payable to: NMACDA

Children's Choir: Use Soprano or Alto.
Youth Choir: Use Soprano, Alto, Tenor or Bass.
